All parents/carers must complete this form. You will need to complete the form in one sitting, and you will need the following information to be at hand:

  • Medical information
  • School details

External Ballet Providers

Students based in Cambridge, Norwich, or those travelling longer distances to Ipswich will usually attend weekly ballet classes with an approved external ballet provider.

If this applies to your young person, please complete the External Ballet Provider Form:

Finance and additional funding

Please do not complete the below forms until instructed by the DanceEast CAT Team. 26/27 grant brackets have not been set by the DfE yet.

NGMD1 – means-tested grant (if your relevant household income is below £74,883.13 before tax)

      You must complete the online form which is sent directly to DfE, this should be completed in one sitting (there is no save & return feature). Please contact us if you need an alternative format.
DanceEast Bursary – (an optional form) you can apply for additional financial help if your household income is above £34,037.12 before tax.
